I am using Darren's mobiles4everyone.com php script. It didn't come with a usable search form so here is the altered search code off my site that works:

<button value="mobile-phones-4u.co.uk" name="site" type="submit"></button>

For the above to work on your site you will have to apply the necessary styling to the form as it will have to use a button image, and also alter the button value to reflect your own site registration url or code. Without the button styling code the submit button will display your site url or registered name. The search is good for looking for phone names.
