In short Opt-In requires some sort of affirmative action by the user, such as typing in their email address on a website to sign up to a newsletter.
Opt-out is where users are automatically signed-up to a newsletter and they have to do some sort of affirmative action to get themselves removed.
Obviously opt-out therefore is somewhat frowned upon.
Double Opt-in or comfirmation opt-in is where the email address of an opt-in request is confirmed by sending an email asking if they wanted to sign-up, only when they respond positively do they get added to the newsletter. This (amongst other things) is to stop other people signing people up to newsletters they don't want.
