PHP Tell A Friend script anyone?

Hi,

I want to use a tell a friend script. I've got one but it
don't seem to work (not passing the data I enter
on the form).

So either I work out how to fix the above, or I ask you if
you know of a PHP tell a friend script that works
(my host runs PHP5 - might this be an issue?)
